CONSENT TO THE PROCESSING OF PERSONAL DATA FOR MARKETING PURPOSES

By submitting this form, I hereby grant Dream Work agency s.r.o., registered office: Štúrova 632/35, 054 01 Levoča, Company ID No.: 56739001, registered in the Commercial Register of the Municipal Court Košice, Section: Sro, File No.: 61410/V, tel.: +421 950 493 554, e-mail: [email protected] (hereinafter referred to as the “Controller”), pursuant to Article 6(1)(a) and Article 7 of Regulation (EU) 2016/679 of the European Parliament and of the Council of 27 April 2016 (GDPR), my consent to the processing of my personal data for the following purpose:

Purpose of processing: to provide information about the Controller, its goods and services, as well as information about promotions, events, and news by sending newsletters to the e‑mail address I have provided.

Scope of processing: name and surname, address, e‑mail address.

Retention period: 2 years from the date of granting consent.

This consent is provided freely, specifically, informedly, and unambiguously. I have been informed that after the expiry of the validity period of the consent or upon its withdrawal, my personal data will be securely destroyed (shredded) and will no longer be processed.

Before granting consent, I have been informed that:

  • my personal data will not be disclosed to third parties or other recipients,

  • the provision of personal data is voluntary and the data will be processed solely for the purpose for which it was obtained,

  • I have the right to request from the Controller access to my personal data, their rectification, erasure, restriction of processing, the right to object to processing, and the right to data portability,

  • I have the right to lodge a complaint with the Office for Personal Data Protection of the Slovak Republic, Hraničná 12, 820 07 Bratislava 27,

  • no cross‑border transfer of data to third countries takes place,

  • no automated decision‑making including profiling takes place.

Withdrawal of consent

I may withdraw my consent at any time, even before its expiry, by e‑mail, telephone, or by sending a written request by registered mail marked “GDPR” to the Controller’s contact details. Withdrawal of consent shall not affect the lawfulness of processing based on consent before its withdrawal.
